The questionnaire has no demographics. Therefore you can’t relate or think of a trend that occurs in certain areas. Also to know there gender or age is extremely important so later on you can make statistics based on age, gender and area.

It would be great to have a mixture of questions so Open questions to collect qualitative answers and use closed questions that will get quantitative. Varied type of answers help build up a more accurate and understandable data.
Have non-biased question means the answer that you receive will be 100% honest meaning the data you receive will be more accurate.
The layout of an questionnaire is very important. Adding a little colour could go a long way. Making the questionnaire visually more interesting but still making sure the questions are clear to read.
